2025 – 2026 Lunch Form: Pre-School to 12th Grade – DAS

Dear DAS Family,

Assalamu ‘Alaikum wa Rahmatullahi wa Barakatu!

Please note that this form is applicable to Pre-school to 12th Grade only .

For payment, kindly submit a check or money order, payable to Darul Arqam. Please include a separate check for each child and write their name in the memo.

The submission deadline is September 25th, 2025. Late forms will not be accepted, so please ensure timely submission.

Please note that lunch will not be served on Bake Sale Wednesdays
